At Stronger Connections Keyworkers, our Early Years Support Coordination service supports families to navigate the NDIS with confidence, clarity, and compassion during the most important years of a child’s development.
We work alongside families of children aged 0–12 years to ensure supports are well coordinated, responsive, and focused on what matters most to your child and family.
What is Early Years Support Coordination?
Early Years Support Coordination helps families understand and implement their child’s NDIS plan, connect with appropriate early intervention supports, and build strong, sustainable networks around the child.
Our focus is on early intervention, capacity building, and empowering families to make informed decisions that support their child’s development, participation, and wellbeing.
How We Support Families
Our experienced Early Years Support Coordinators will:
Explain your child’s NDIS plan in clear, easy-to-understand language
Support you to identify goals that are meaningful for your child and family
Connect you with suitable early childhood providers and community supports. We have strong and long-standing relationships with many Gold Coast providers.
Coordinate services across health, education, and community settings
Support collaboration between therapists, educators, and caregivers
Monitor supports and funding to ensure they are being used effectively
Assist with plan reviews, reports, and evidence when required
Reduce overwhelm by being your main point of contact
Family-Centred and Strengths-Based
We recognise that families know their children best. Our approach is family-centred, strengths-based, and respectful of each family’s unique journey.
We work at your pace, value your voice, and aim to build confidence and skills so families feel empowered — not overwhelmed — by the NDIS.
